When you are finished filling out the form you can either scan it and email it to [email protected]
or
drop it off at the Blue Ox Equipment garage on Highway N.
This form is used for both ATV and Snowmobile trail maintenance and includes ATV Grooming.
Please use the form with the revision date of 8-20-2026.
